    Importance of Clinical

    Documentation in Value Based Care

    Tuesday, April 7, 2026

    12:00PM - 1:00PM EST

Presented By: Mona Bambha, MD, CPC, CRCD

Founder & President - CoDoc Academy

    Session Description:

Proper documentation of a patient’s medical records has always been a crucial component of patient care. In today’s healthcare landscape, this importance has intensified, as government reimbursements are increasingly tied to the quality and accuracy of providers' documentation.

Accurate documentation depicting the accurate disease burden of the patient population is an extremely important pillar in risk adjustment and value-based care. It's the most effective way of communicating the needs of our patient population.

During this presentation we will dive into understanding the importance of coding and documentation in risk adjustment process and the risk adjustment score and how to efficiently navigate through this.

Effective transitions of care are critical, high-risk periods where fragmented communication leads to poor outcomes. This presentation highlights the vast range of information available in real time and how that information flows among members of the patient’s care team to evolve from a fragmented, disease-focused discharge to a unified, patient-centered care plan that addresses each patient’s complete needs - clinical, behavioral, and social.

New York’s recent updates to its SHIELD Law broaden protections for reproductive health information by fortifying safeguards against disclosure of sensitive patient data, including limiting the release of private medical records and tightening restrictions on out of state subpoenas seeking such information.
